These all sound like serious software glitches to me, at least in some cases. Note the part where BMW denies any problems with the car. They might well be right, as it's quite possible that the driver simply panicked, lost control of the car and crashed, so to deflect blame away from herself, said the car did it, but the evidence says otherwise.
